Phone Order Form

ALL info for phone orders, except for credit card number and expiration date, must be submitted through this form. Once submitted, your credit card info only will be collected by phone.

Please read the following carefully, or your order will most likely be delayed or canceled:

• Prices only include shipping on orders paid online or by mail--not by phone. You must contact us for a shipping quote before filling out this form, or you can order online instead and avoid the shipping charges. Orders submitted without the correct shipping will not be processed.
• Orders paid by phone can only be shipped to the billing address of the credit card. If shipping to a different address, you must pay online or by mail, following the directions for adding an authorized shipping address.
• Orders can only be paid by phone in the continental US.
• Only credit card info is collected by phone--nothing else. Any other questions need to be addressed online before submitting this form.
• You must use the price quote form for any questions or for checking stock & pricing. Inquiries sent through this form will be deleted. Orders only.
• Orders cannot be changed by phone. Any changes or additions must be done online before paying by phone.
• Phone orders are only processed Tuesday through Thursday. Internet orders receive priority, so phone orders might not ship the same day they're placed. Please order online instead for quicker processing and to avoid shipping charges.

You can avoid shipping costs & delays by placing your order online with our secure order form, or by mailing your order in.

We'll call for your credit card info ASAP and process your order, but please note that internet orders receive priority.  Phone orders aren't guaranteed to be processed before the shipping cutoff time of the day they are placed.
